FILL OUT PRE-SCREENING FORM

After booking an appointment, you will be receiving 2 email reminders, one 48 hours before your appointment and the other, 24 hours before your appointment. The first email contains a SERVICE LIABILITY WAIVER which must be filled out prior to you coming into your appointment. The second email contains a COVID-19 pre-screening form which must only be filled out on the day of your appointment. 

RESCHEDULE IF SICK

Please reschedule your appointment if you are sick. This includes if you are showing any symptoms and you think you're sick. If you show up sick, I will send you home and you will be charged a $30 fee.

NO GUESTS ARE ALLOWED

My studio is a small space and due to COVID-19 regulations I can only accommodate a maximum of 2 people. 

BEFORE YOU ARRIVE TO THE STUDIO:

  • Let me know when you arrive to the studio and wait until I let you know you can come in

  • Arrive wearing a face mask

  • Bring minimal belongings with you

 

WHEN YOU ARRIVE AT THE STUDIO:

  • Your temperature will be taken at the entrance

  • Put your belongings in the sanitized container

  • Wash your hands

  • Make sure you have filled out pre-screening forms beforehand

RESCHEDULING/CANCELLATION 

If you need to reschedule or cancel your appointment, please contact me via email at least 48 hours before your appointment to avoid a late rescheduling/cancellation fee. Only one (1) rescheduled appointment is allowed per appointment, unless there is an extenuating circumstance. Rescheduling fee will apply if you need to reschedule more than once.​

LAST MINUTE CANCELLATION/RESCHEDULING

If you cancel or reschedule your appointment with less than 24 hours notice your card on file will be charged a fee of $30. ​The fee must be paid in order to book with us again.

NO SHOW

If you do not show up to your scheduled appointment time you will be charged for the full service to your card on file (for ombre powder brows, you will be charged 50% of the procedure) and will be prohibited from booking with us again.

TARDINESS 

Please let me know if you will be arriving late well beforehand and/or as soon as you know. If you are over 15 minutes late, your appointment may be cancelled depending on what my day looks like. In some cases where we are still able to take you after being more than 15 minutes late, alternate arrangements will be made. You will be charged $30 for the late fee and it must be paid before we start your service. In the event you decide to not show up, it will count as a no show and you will be charged accordingly. 

PAYMENT

A non-refundable 20% deposit is required to secure your booking. Your deposit will count towards the total cost. Outstanding balance is due in cash or interac at the initial appointment. Listed pricing doesn't include HST. ALL bookings are final and total fees are non-refundable. An invoice totalling your remaining balance will be sent to you via email prior to your appointment with us.

ALL PAYMENTS ARE NON-REFUNDABLE AND NON-TRANSFERRABLE.

As a personal care service provider, my product is my work on you. If you are experiencing any problems please let me know AS SOON AS POSSIBLE AND WITHIN 24 HOURS so I can help you. *If you do not contact me within 24 hours, there will be an applicable charge should we need to fix anything.
